  • Adhere to our Insurance and Billing Policy and Practices
The patient has a right to:
  • Review the record of your case at any time
  • Refuse all or part or your treatment and be informed of the expected results of those decisions.
  • Be informed of any relationship of this practice and any other organization or service to which you are referred. You have the right to choose the organization or service you prefer.
  • Receive service without regard to race, religion, color, sex, national origin, religion, age, or handicap.
